Your urban forest management plan should have a title page. At a minimum, the title page would include the title, prepared by, prepared for, and the date of the plan. If you are preparing a plan for a local government it is possible that they will have a standard title page format the you should follow.

The title page for a plan for a city plan may include a list of the city council members and city departments or staff taking part in the plan preparation. If not, it may be appropriate to include a seperate Acknowledgements section listing people who contributed to the writing of the plan. City plans may also include the date of plan approval by the city council.
